py for the investigation of lower abdominal pain. Colonoscopy showed a whitish, flat, elevated polyp, 10mm in diameter, in the ascending colon (l" Fig. 1). Endoscopic mucosal resection (EMR) was carried out, and the resected specimen histologically revealed both invasive cancer and serrated adenomatous glands coexisting in an inverted hyperplastic polyp (IHP) (l" Fig. 2a–c). A follow-up colonoscopy 6 months later revealed an irregularly elevated normal mucosa with fold convergence adjacent to the EMR scar (l" Fig. 3). Although endoscopic features suggested a recurrent cancer, endoscopic biopsies unfortunately failed to identify recurrent cancer histologically. Surprisingly, a second follow-up colonoscopy only 3 months later showed an advanced colon cancer (l" Fig. 4). Endoscopic biopsies showed poorly differentiated adenocarcinoma, and therefore surgical resection was carried out. Histologically, the recurrent cancer was a subserosally invasive, poorly differentiated adenocarcinoma with vascular invasion and nodal involvement, and it was classified as stage III (T3N2M0). This IHP was 10mm in diameter, which is a so-called large hyperplastic polyp, and interestingly it demonstrated a characteristic endoscopic and histologic feature of IHP [1]. IHP is a variant form of hyperplastic polyp with a characteristic histologic feature of epithelial misplacement or inversion of the epithelium into the submucosa [2]. Our case was unique and the histology detected the presence of not only an invasive, poorly differentiated carcinoma but also serrated adenomatous glands surrounded by hyperplastic glands. To date, three cases of IHP associated with adenoma have been reported [3]; however this is the first case report of both of invasive cancer and serrated adenoma arising in an IHP. Recently, serrated polyps have been histologically defined as polyps that demonstrate a serrated or “sawtoothed” appearance, and include hyperplastic polyps, serrated adenoma, and Invasive cancer arising in a colonic inverted hyperplastic polyp

Invasive adenocarcinoma arising from a mixed hyperplastic/adenomatous polyp and synchronous transverse colon cancer
An admixture of hyperplastic and adenomatous components within the same polyp is unusual. Adenocarcinoma arising from a mixed hyperplastic/adenomatous polyp (MHAP) occurs even more rarely. We report the first case of a 59-year-old male who presented with invasive adenocarcinoma originating from a MHAP at a sigmoid colon and synchronous transverse colon cancer.

Background and purpose: Colorectal cancer is one of the most common types of cancers worldwide. Most of the times it is superimposed on an adenomatous polyp. Today, colonoscopy is the gold standard screening test for diagnosis of colorectal cancer which prevents developing cancer on the adenocarcinoma sequence path way. The goal of this study was to evaluate the gross and microscopic characteri...
متن کاملA Case of Gastric Inverted Hyperplastic Polyp Found with Gastritis Cystica Profunda and Early Gastric Cancer
A gastric inverted hyperplastic polyp is a rare type of gastric polyp and is characterized by downward growth of a variety of mucosal components into the submucosa. The polyp consists of columnar cells resembling foveolar epithelium and pyloric gland epithelium and can coexist with gastritis cystica profunda. Frequently, adenocarcinoma can coexist, but the relation is not clear. A 77-year-old m...
متن کاملIntramucosal Colon Cancer Developing in an Inverted Hyperplastic Polyp
Inverted hyperplastic polyps (IHPs) of the colon are unusual variant of exophytic hyperplastic polyps that are characterized by epithelial displacement or inversion of the epithelium into the submucosa. To date, only a few cases of IHPs associated with adenoma or adenocarcinoma have been reported. However, due to the rarity of these polyps, their malignant potential is poorly understood. A 56-y...
متن کاملMultifocal Adenocarcinomas Arising within a Gastric Inverted Hyperplastic Polyp
We present herein the occurrence of multifocal adenocarcinomas with a minute signet ring cell carcinoma that arose within a gastric inverted hyperplastic polyp (IHP) in a 40-year-old woman. Endoscopic ultrasonography demonstrated a heterogeneous hypoechoic mass in the third layer of the gastric wall. The endoscopic submucosal dissection specimen measuring 3.5×3.2×1.8 cm was a well-circumscribed...
